    1. This form is used by Emanuel County Fire and Rescue to record details of hazardous materials incidents only. It captures information such as date, time, location, product details, response actions, responding apparatus, and a narrative summary.

      Additional reporting may be required for hazardous materials incidents. Be sure to complete all necessary state and federal documentation in addition to this form.
